Support Coordination aims to help people with disability and their family, or support network, to work their way through their National Disability Insurance Scheme (NDIS) journey, and achieve their NDIS goals.
Support connection
This support is to build your ability to connect with informal, community and funded supports, enabling you to get the most out of your plan and achieve your goals.
Support coordination – coordination of supports
This support will assist you to build the skills you need to understand, implement and use your plan. A Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, live more independently and access your community.
Specialist support coordination
This is a higher level of support coordination for people in complex situations A Specialist Support Coordinator will assist you in managing challenges in your support environment, ensuring consistent delivery of services.

Assistance with Daily Living/

Connectx Support Services
At home, in the community, for health or leisure, we provide one-on-one support, seven days a week.

Support getting ready for your day
To get out and about in the community
Help doing household chores
Help with maintaining your garden
Assistance with food preparation and meals
Support with health and fitness
Transport to appointments/other commitments
These are just a few things we can assist with, tell us about your goals so we can tailor support to meet your needs.
With male and female staff of all ages, fluent in English, Mandarin Chinese (普通話), Cantonese (廣東話), Vietnamese, Punjabi and Hindi, we are sure to have someone that is the right fit for you.
*Accepting of funding that is Plan-Managed or Self-Managed.

We remove the administration burden of paying your service providers and dealing with the paperwork, while keeping you aware of the funding in your plan at all times. You will still have total control of your NDIS plan, while giving you more time to live the life you want to live!
